Consent from for City Hearing Log app

By accepting to use the City Hearing Log app you declare that:
I consent to the processing of:
• My user registration/authentication data (pseudonym and password) that I will provide when creating an account to the City Hearing Log application;
• My smart-phone data (GPS and sound record);
• The descriptive text (annotations) that I will provide during the acquisition of sound;
• The sounds that I will collect through the camera integrated in my mobile device;
• Other information relevant to the data that I will collect (responses to a questionnaire, reporting potential events and/or providing general comments)
so that I can use the City Hearing Log app.
I consent to the maintenance of my personal data until December 31, 2022:

• My smart-phone data (GPS and sound record);
• The descriptive text (annotations) that I will provide during the acquisition of sounds;

Privacy Policy for City Hearing Log app
Introduction
In the context of the European Union’s Horizon 2020 Programme for Research and Innovation, the project City Hearing Log has received funding under Grant Agreement No. 688930

The project aims to investigate how citizens may help to improve monitoring critical environmental phenomena such as floods and their potential consequences by providing timely information on the land-use changes of their environment. This is done through low-cost equipment that most citizens have or may easily obtain (e.g. their smartphones) and through taking photos of their environment at regions that are really important for assessing flood (e.g. images of forest areas being burnt or converted to crops, images of river banks that are filled with debris). These images are then used for improving land-cover and land-use maps with updated and detailed information and for improving flood modelling. The performance and accuracy of the SCENT toolbox will be validated in two large scale demonstration campaigns that will be executed in parts of the Kifisos river basin in Attica, Greece and parts of the Danube Delta region in Romania.
Purpose of data collection
This particular activity to which you are invited to participate today relates to a pilot thematic campaign towards the collection of environmental-related information (such as sounds of the city).
During your registration to the City Hearing Log app, you will be requested to provide a pseudonym and password. This information is needed so as to allow you to use our app and will be stored along with the collected data, whilst it will not be used somehow by the project. The sounds you will take and text that you will provide describing what you have recorded will be captured and stored during the demonstration. The same will happen with your location in order to associate the captured sounds with specific geographic coordinates. The data that you will collect, will be processed by City Hearing Log consortium partners listed below and will be made available to other partners of the consortium so as to achieve the research objectives of the City Hearing Log project explained to you in this information sheet.
Types of data collected
The types of data collected in the context of this activity can be summarized as follows:
➢ Data related to the user registration/authentication: This refers to the pseudonym and password that you will be requested to provide when you create an account to the Scent Explore application;
➢ Smart-phone data: This refers to location data (GPS) that will be collected from the sensors embedded in your mobile devices during the use of City Hearing Log app;
➢ Annotations related to the sounds: This refers to the descriptive text (annotations) that you will provide during the acquisition of sounds;
➢ Sounds: The data that you will collect through the microphone integrated in your mobile devices;
➢ Other information relevant to the collected data: While using the Scent Explore application, you may require to respond to a questionnaire and/or to report potential events of interest in the area where you collect the data and/or to provide your general comments on the content of the images or on the surrounding area. These data will be collected through the Scent Explore app and will be used in the context of the research activities of the project.
Exercise of your rights
It is noted that according to the General Data Protection Regulation (Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016), you may exercise the following rights that derive from the Regulation:
• Right of access and right to rectification for inaccurate personal data
• Right to erasure of personal data if they are not necessary for service provision
• Right to restrict processing of your data
• Right to object to the processing of your data
• Right to withdraw your consent to processing of your data
• Right to data portability, namely right to receive your data in a structured, commonly used and machine-readable form so that they can be transferred to another data processor.
• Additionally, you have the right to submit a written complaint to the responsible supervisory body for personal data protection in each country.
The General Data Protection Regulation also gives you right to lodge a complaint with a supervisory authority, in particular in the European Union (or European Economic Area) state where you work, normally live or where any alleged infringement of data protection laws occurred.
Consent
By accepting this privacy policy, I confirm that:
1. I have read, understood and accepted the information about the project, as provided in the City Hearing Log Information Sheet.
2. I have been given the opportunity to ask questions about the project and my participation.
3. I voluntarily agree to participate in the project.
4. I am aware of the data that will be collected during this activity.
5. I understand I can withdraw my consent regarding my participation in the project any time without specific reasons. Such withdrawal shall be given as easy as I gave my consent. I understand I will not be penalised for withdrawing my consent nor will I be questioned on the reasons of such withdrawal.
6. The procedures regarding confidentiality have been clearly explained.
7. If applicable, separate terms of consent for interviews, audio, video or other forms of data collection have been explained and provided.
8. It is clear to me that my data will be processed with regard to research, publications, sharing and archiving purposes. I understand that at the end of this project my data will either be destroyed or reused, only upon my consent.
9. I am above 16 years old and no parental consensus is required
10. I agree my name to be included in reports, publications and other research outputs of the project as well as any other information I have provided.
11. I agree to sign and date this informed consent form.
